Prime 4 Channel 2 audio always leaks to channel 4 – fader 2 non-functional

when any input is flowing through channel 2, the audio does not come out of this channel regardless of channel 2 fader position. However, if the channel 4 fader is up i can hear the audio from channel 2. Summarized, the signal going into channel 2 always leaks into channel 4. Fader 2 is essentially useless as it do not control the audio levels for this channel. The only way to hear audio from channel 2 is by making sure the channel 4 fader is turned on.

I have tried many things in attempt to remedy this issue. I have upgraded and downgraded firmware and even did a reset on the whole device.

Please let me know if there are any other steps I should try to remedy this.
